Amida Care Live Life Advantage (HMO SNP) is a Medicare special needs plan for those diagnosed with HIV/AIDS. When you choose this plan you typically have low monthly premiums.

Low Monthly Plan Premiums

When you choose Amida Care Live Life Advantage, you have no medical care premium.* You will have a monthly drug premium of $36.90 (In 2016, the monthly drug premium will be $39.70). However, if you qualify for Extra Help, your premiums, co-pays, coinsurance, and deductibles may vary based on the level of Extra Help you receive. Please contact the Plan for further details. If you are eligible for Medicaid, you also may receive assistance with out-of-pocket medical expenses.

  • Medicare covered home health care services
  • Outpatient diagnostic and therapeutic radiologic procedures and tests
  • Routine transportation to/from plan-approved location
  • Emergency response alert device
  • Preventive Services
  • Dentures every 5 years

PLUS $0 co-pay for:

  • Dental and vision
  • Home health service
  • Nursing hotline

PLUS we cover:

  • Monthly allowance to spend on covered over-the-counter health items
  • Primary care doctors and specialist visits
  • Hospital care
  • Lab services and diabetic supplies
  • Skilled nursing facilities
  • Ambulance service
  • Emergency service
  • Cardiac rehabilitation services
  • Chiropractic services
  • Physical and occupational therapy
  • Mental health care and other services.
